The preset data that needs to be saved is correct, but the displayed icon is wrong.
It shows him wearing earrings that don’t belong to him.

Not sure it’s the same issue or not, but I’ve seen stuff like that in the game too, not just CAZ. A schoolmate went from adolescent to teenager, and the game gave him a child face in the icon (e.g., on the friends list), that looks nothing like the taller face on a teenager. And it gave him a red cap in the icon, that I’ve never seen him wear even when throwing basketballs at the sports ground. (But, strangely enough, looks like the cap my zoi has in her sports outfit.)
So yeah, occasionally the game pulls stuff from somewhere else.

Happens in CAZ for me too.
An observation:
The icon picked up earrings from the zoi that was previously saved as a preset in the same game session.

Yes, unfortunately this Zoi portrait bug is a real drag lately. Didn’t know that was happening with presets too. Hope they’ll investigate and find a solution for it soon.
